Waiver, Photo Release and Sexual Harassment Policy
We understand and agree that all participation in any fitness program or use of the school's facilities or
equipment on or off the premises of the school shall be at our own risk. We hereby release, indemnify and
hold harmless Iron Roots BJJ (Iron Roots Brazilian Jiu Jitsu dba Iron roots BJJ) and it's officers, directors,
employees and agents, from and against any and all claims, demands, damages cost and liabilities of any
kind or nature including attorney's fees and costs for injury to or death of myself or of any person who
became entitled to use of the facilities of the school which arise directly or indirectly out of or in connection
with our participation in any program or course of instructions either on or off the premises of the school or
by virtue of our presence at the school or at any of the schools off premises events whether or not in fact we
or such other person are then participation in any particular program or event.
We understand and agree that the school shall not be responsible for the conduct of other users of the
school or its facilities or conduct and we shall not bring any action or proceeding against the school for any
payment compensation or claim for any loss of life or injury caused by any such user.
I (and/or my child) fully recognize the risk of injury and or illness inherent in participating in any fitness or
martial arts program and we represent to the school that we have taken all reasonable steps to determine
and here by warrant that we are in good health and physically capable of participating in the programs and
courses of instruction offered by the school. We acknowledge that the school shall make no and shall have
no responsibility to make any independent evaluation of our physical health or fitness.
PHOTO RELEASE
I hereby grant Iron Roots BJJ permission to use my likeness in a photograph, video, or other digital media
(“photo”) in any and all of its publications, including web-based publications, without payment or other
consideration.
I understand and agree that all photos will become the property of Iron Roots BJJ and will not be returned.
I hereby irrevocably authorize Iron Roots BJJ to edit, alter, copy, exhibit, publish, or distribute these photos
for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my
likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the
use of the photo.
I hereby hold harmless, release, and forever discharge Iron Roots BJJ from all claims, demands, and causes
of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my
behalf or on behalf of my estate have or may have by reason of this authorization.
SEXUAL HARASSMENT POLICY
Iron Roots BJJ prohibits discrimination of any kind, including sexual harassment by instructors, fellow
students and any other participant at Iron Roots BJJ.
Sexual Harassment is defined as any harassment based on a person’s sex, such as unwelcome sexual
advances, requests for sexual favors, and other verbal or physical harassment of a sexual nature, including
offensive remarks about a person’s gender. Both males and females can be victims of sexual harassment.
Prohibited behavior includes any behavior that creates an offensive environment (Hostile Environment) or
that results in an adverse imbalance of power such as the victim being denied advancement or assistance
(Quid pro Quo). Some examples include:
A higher ranking student telling a lower ranking student that they will advance quicker if they
provide special favors.
While grappling one partner touches the other longer than is necessary or in a caressing manner.
Texting another student unsolicited nude photos.
These examples are not all-inclusive.
Individuals violating this policy will be subject to discipline, up to and including permanent expulsion from
the school.
Students, regardless of gender, should report any instances of sexual harassment to the owners of Iron
Roots BJJ. The report will be investigated and the individual doing the harassment may be asked to leave.
Insofar as possible, reports will be maintained as confidential in order to prevent retaliation.
